Gather once, apply everywhere
The documents checklist
Most programs ask for the same core documents. Bring these to your appointment — or as many as you can find — and we'll handle the rest.
Identity — for every household member
- Photo ID (driver's license or state ID) for adults
- Social Security cards for everyone in the household
- Birth certificates for children
- Immigration documents, if applicable (green card, visa)
Income — usually the last 90 days
- Pay stubs for the past 30–90 days (all jobs, all earners)
- Self-employment records or most recent tax return
- Social Security, SSI, SSDI or pension award letters
- Unemployment, child support, or other benefit statements
Household & housing
- Lease agreement or mortgage statement
- Most recent property tax bill (homeowners)
- Proof of address (a current bill with your name and address)
Bills & expenses
- Current gas, electric, water and sewer bills (if you pay them)
- Childcare expense receipts or statements
- Medical bills or proof of medical expenses
- Health insurance cards and premium statements
Good to know
- Missing a document? Come anyway. We can often request replacements (Social Security cards, birth certificates) as part of the process.
- Most programs require income re-verification every year — keep this folder somewhere safe.
- Never email Social Security numbers or full account numbers. Bring documents in person or ask us about secure options.
